END OF LIFE SERVICE

Providing an individually tailored, compassionate service to support carers at this difficult time, with the aim of enabling the person with care needs to remain in their own home for as long as possible.

Referrals can be accepted from any source, but should be for those caring for patients on the palliative care register with their GP (i.e. in the last six months of life)

One of our professionally trained Carer Support Workers can visit for up to 8 hours per week to take over caring tasks (such as personal care, administration of medication, meal preparation) and can also provide practical and emotional support.  Regular breaks can be provided during the day, evening, weekend.

We are registered with the Care Quality Commission.

Our service supplements and complements, but does not replace existing services.  We will work alongside the community nursing teams from the hospice and district nurses and to support people to stay at home.

We aim to provide support to carers at this difficult time so that hospital or hospice admission, arising from carer exhaustion or stress, can be avoided

The service is supervised by our professional Macmillan Liaison Officer and is funded by Croydon PCT.    As such, please note that eligibility criteria apply to this service and that in some cases, funding may not be available.

If you have a Social Services Care Manager – you may be able to ask them to fund some of your respite needs or ask to receive direct payments so that you can purchase our support direct.
